Position Summary

We seek a compassionate and skilled Home Health Occupational Therapist to provide therapeutic services to patients in their homes. This role focuses on helping individuals regain independence and enhance their quality of life. The ideal candidate will possess strong clinical skills, a patient-centered approach, and the ability to work independently.

Qualifications

Accredited Occupational Therapy degree
Active state licensure as required in Nevada
CPR certification
Strong verbal and written communication skills
Valid driver’s license, insured automobile, and safe driving record

Essential Functions

Conduct assessments, diagnostic tests, treatments, and evaluations as per the medically approved Plan of Care.
Develop individualized treatment programs with clear goals for improved function.
Accurately document findings, treatment plans, interventions, and patient progress.
Collaborate with physicians and agency staff to ensure alignment on care plans and necessary modifications.
Guide therapeutic self-care activities to patients.
Educate patients, family members, and agency staff on treatment procedures.
Recommend, design, and order adaptive/assistive devices.
Maintain compliance with agency policies and professional standards.
Participate in care conferences and interdisciplinary coordination efforts.

Supervision of Occupational Therapy Assistants

Provide instruction and oversight to Occupational Therapy Assistants (OTAs).
Remain accessible by phone for guidance and support.
Conduct on-site supervision every four to six visits, minimally once a month.
Perform functional assessments and review Plans of Care during on-site visits.
